How do I get my unique ID to login to the online health risk assessment?
Your HRA unique ID number can be requested from Human Resources at (517) 796-8669 or by emailing Amanda Patterson.
How many times can I take the online health risk assessment?
You can only take the online HRA once. If you want the health risk assessment to be the most accurate, get a complete physical exam from your doctor before you take the HRA and then use the results from the physical to input into the HRA.
What are the 2008 health coach session dates?
