Step 1: Health Risk Appraisal
- Personalized health report online

To request an account for the HRA, contact Human Resources at (517) 796-8460 or email thomsennicolea@jccmi.edu The turn-around time for an account build is roughly one week.
Step 2: Health Screening
Please choose either Option 1 or 2
Option 1: Take the above attached document labeled Physician Screen Form to your Physician, and have the listed tests completed by the dates listed above. In most your insurance will cover the cost as a part of you annual physical. (If you have had these tests completed within the last five months, you simply need to obtain your numbers). Bring your results with you to your Health Coaching session (step 3). Please do not send or give HR a copy of your results, as we do not want or need them.
Option 2: Contact Nikki Thomsen in HR to schedule an appointment to have your lab work completed on campus, by an Allegiance Health Representative. Please note that these labs do require you to fast.
*JCC does not need or want copies of your lab results; please rest assured that JCC will only receive non-identifying information from Allegiance Health.
Step 3: Health Coaching
15-20 minute individual consultation with a trained specialist in exercise nutrition, nutrition, tobacco cessation, and health behavior change.

Your name and a unique identification number are required to confirm your eligibility to take advantage of this Health Risk Appraisal (HRA). Beyond this purpose, your information is considered anonymous. Your data are held in confidence by the University of Michigan Health Management Research Center and are used in an aggregate, anonymous form for reporting and scientific research.
Before You Begin Your HRA
The health risk appraisal is a research-based, validated tool used to assess your health risks and behaviors. The more detailed and specific the information is that you put into the HRA, the more accurate your personalized health report will be when it is generated. With that in mind, if you’ve had a physical exam in the past year, you can ask your doctor for the results, including your total cholesterol, LDL cholesterol, HDL cholesterol, triglycerides, glucose, blood pressure, and body mass index. This information will increase the accuracy of your health report.
