AGREEMENT OF COOPERATIVE SERVICES FOR A

DIAGNOSTIC MEDICAL SONOGRAPHY PROGRAM

BETWEEN JACKSON COMMUNITY COLLEGE AND

LAKE MICHIGAN COMMUNITY COLLEGE

  

THIS AGREEMENT made this June 6, 2005, between Lake Michigan Community College, 2755 East Napier Avenue, Benton Harbor, Michigan 49002-1899 (hereinafter referred to as “Lake Michigan”), and Jackson Community College, located at 2111 Emmons Road, Jackson, Michigan 49201 (hereinafter referred to as “Jackson”).

 

·                      WHEREAS, both colleges desire to make available Diagnostic Medical (DMS) programs to their students, and

 

·                      WHEREAS, both colleges are community colleges organized under the laws of the State of Michigan and accredited by the North Central Association of Colleges and Schools, and

 

·                      WHEREAS, both colleges desire to provide educational services to their students in the most economic way, and

 

·                      WHEREAS, both colleges recognize the community and professional need for Diagnostic Medical Sonography personnel, and

 

·                      WHEREAS, Jackson does provide Diagnostic Medical Sonography programs, and

 

NOW THEREFORE, for and in consideration of the premises and the mutual covenants and agreements herein contained, the parties hereto agree to as follows:

 

1.                    Lake Michigan Community College agrees to:

a.        identify students who have completed the courses required as prerequisites for Jackson’s Diagnostic Medical Sonography program; and will refer these program applicants to Jackson for final selection and program orientation.

b.        offer academic courses for those students at Lake Michigan as follows:

c.           advise students to register for DMS 100 Introduction to Diagnostic Medical Imaging; DMS 104 Introduction to Sonographic Instrumentation and HOC 130 Introduction to Health Occupations, at Jackson.  Jackson will allow students to register for these three courses via MCCVLC registration and pay the established MCCVLC rate. These courses are prerequisites offered at Jackson and need to be completed prior to being considered for admission to a DMS program.

 

d.            advise students that they will need to complete the courses listed in 1. b. and c. prior to applying for admission to any JCC DMS program. Admission into a JCC DMS program is not assured. Lake Michigan students will need to follow the outline of this agreement to become eligible to apply for admission into a JCC DMS program. Admission into JCC DMS programs are competitive.

 

e.           advise students that if admitted and assigned to a DMS clinical program that they shall be considered students of Jackson Community College and will be subject to and governed by any and all applicable rules of Jackson Community College.

 

 

2.                 Jackson Community College agrees to:

a.            accept no more than the total maximum allowed number of students that the Joint Review Committee on Education in Diagnostic Medical Sonography recommends for hospitals, clinics, and labs or any other Diagnostic Medical Sonography Clinical Practicum Affiliate in the Lake Michigan service area that would join in agreement with Jackson for this program. Admission into a JCC DMS program is not assured. Lake Michigan students will need to follow the outline of this agreement to become eligible to apply for admission to a JCC DMS program. Admission to JCC DMS programs is competitive.

b.           will consider DMS program applications and fees of qualified Lake Michigan students involved in this agreement who have met the requirements for admission.

c.           will provide specific Diagnostic Medical Sonography course work (including clinical practicum courses), for Lake Michigan students accepted into the DMS program for up to forty two (42) credit hours.

d.          advise Lake Michigan students who are accepted into the DMS program to take Diagnostic Medical Sonography courses at Jackson’s out of county rates.

e.         students will participate in clinicals at hospitals/clinics associated with their service district for the purpose of DMS clinical education for the following Jackson courses:

 

DMS-122 DMS Clinical Experience I             6 credits

DMS-223 DMS Clinical Experience II           6 credits

DMS-224 DMS Clinical Experience III          6 credits

 

for a total of 18 credits     or

 

DMS – 143 Echocardiography Clinical I          3 credits

DMS – 147 Echocardiography Clinical II         5 credits

DMS – 242 Echocardiography Clinical III        7 credits

 

for a total of 15 credits     or

 

DMS – 161 Vascular Clinical I            4 credits

DMS – 265 Vascular Clinical II           4 credits

DMS – 266 Vascular Clinical III          4 credits

 

for a total of 16 credits

 

f.           will accept all satisfactorily earned credits, as indicated in Section 1.b. taken either at Lake Michigan and Jackson and will accept all satisfactorily earned credits, as indicated in Section 1.c. and 1.d. taken at Jackson.

g.           will grant the appropriate degree or certificate to all Lake Michigan students who complete the requirements

h.            will provide overall program supervision by the program coordinator.

i.             all clinical affiliation agreements will be between Jackson Community College and the clinical site. Lake Michigan will support the communication of these agreements and will assist Jackson staff to identify potential clinical sites.

j.             will apply for programmatic accreditation through the Commission on Accreditation of Allied Health Educational Programs (CAAHEP) in cooperation with the Joint Review Committee on Education in Diagnostic Medical Sonography.

3.     Termination

a.           this agreement may be terminated at any time by either party by written notice given to the other party in writing at least ninety (90) days prior to the effective date of such termination; provided, however, that Lake Michigan students that have been admitted to the program at Jackson at the time of the notice be allowed to finish the entire program at Jackson even if the students have more than one year (but not to exceed two years) left in the program.

b.          this agreement will be reviewed and revised as necessary but shall remain in effect until termination as specified by the Termination clause of this agreement.

c.           this agreement is binding on all successors and assigns of Lake Michigan and Jackson.

IN WITNESS WHEREOF, Lake Michigan and Jackson have executed this agreement as of the day and year listed below.
