Student Directory Information

Directory information includes name, address, and Social Security number. To change or correct student directory information we require valid proof of the new information. Valid identification is one that is current (not expired) showing your current/new name and current address.

Examples of valid identification include:

  • Driver’s license
  • State or employer issued ID card
  • Marriage or divorce papers
  • Social Security card
  • IRS tax returns (block out financial information)
  • Other official court documents

Updating Your Name

Valid identification is required to change your name on an educational record; a copy of this documentation must be kept on record at the College and your legal signature is required.

Updating Your Address

You must also submit proof following the guidelines listed below:

  • Valid identification is required only when your new address reduces your tuition rate. This means moving from outside to inside Jackson County or the State of Michigan.
  • Residency may also be verified with documents showing your county of residence (voter registration card, vehicle registration, dog license, property tax receipt, or rental/lease agreement; a copy must be kept on record at JC).
  • Two pieces of identification are required to verify your address, must include your name, and must not be expired. Your county of residence must be indicated on at least one of these documents.

Updating Your Social Security Number

You must also submit one of the following:

  • A signed copy of your Social Security card
  • A copy of a recent tax return (block out financial information)
  • A pay stub showing your Social Security number (block out financial information).

Use this form to update your personal information with Jackson College, including name, address and social security number.
