If you have witnessed or suspect any misconduct within our institution, we encourage you to report it. Your vigilance helps ensure a safe and ethical environment for all. Types of Misconduct Fraud Abuse of power or resources Corruption Illegal activities Immoral conduct Illicit activities Any activity posing a danger to public health and safety Additional Reporting This incident form is specifically designed for reporting egregious violations of law or college policy. For other types of complaints or violations, please utilize additional forms provided below. Housing Violations Housing regulation violations or violations of the student code of conduct can be reported utilizing the Incident Reporting Form. Sexual Harassment Sexual Harassment should be reported to the College’s Title IX Coordinator. Academic Complaints Academic Complaints (grade disputes, financial aid appeals, etc.) should be reported to the Student Resolution Advocate. File a Report or Complaint Submissions are routed to an external third party to ensure impartiality and objectivity in the investigation process. This form should be utilized only for egregious violations of law or college policy that can not be reported utilizing internal procedures.
